Melhorn & Melhorn, D.O., Inc. participates with the insurance companies listed below.  
Although we do participate with these carriers, we advise you contact your insurance 
company with any benefit, authorization, referral, deductible or co-pay questions prior 
to your visit.  As a courtesy to our patients, we will file participating insurance claims. 
Because certain services in this office are considered physical therapy by insurance 
companies, it is the patient's responsibility to know your benefits and whether 
preauthorization is required.  Please be prepared to pay any insurance co-payments 
due at the time of service.  Denied services will be billed to you accordingly.  
If your insurance company is not on the list below, we will still file the claim as a courtesy.
However, please be advised they may reduce their payment, apply to deductible or deny it
completely. For patients with no insurance, payment is expected at the time of service. 

In the event you were in an auto accident, we do not file third party claims 
and do not wait for settlement of claim to be paid.  We will file your applicable 
health insurance.  Written authorization is required to release any information related 
to the accident. 

We ask your cooperation in updating your demographic and insurance 
information yearly and advise us of insurance changes.  If you have any questions,
